• Must complete the evaluation and purchase clock hours within one year.
  • A total of 50 clock hours are available. Clock hours are 3-5 clock hours $10.00 then $2.00 per clock hour.
  • SI facilitator will go in and mark participants attended after the last day of the class.
  • Participants will receive an email asking them to submit the evaluation.
  • Participants will have one year to complete and submit the online evaluation and to purchase their clock hours online.
